In a story on Page 3 of Thursday's Hampton Roads section ("Man dies after hitting bus head-on"), the name and age of the man killed in an accident Wednesday involving a minivan and a school bus were incorrect. The victim was Fredrick Michael Adkins, and he was 54.

Because of a source error, a story on Page 11 of Wednesday's front section misstated the type of workers who would lose their jobs at Farm Fresh. No information technology positions are included in the planned layoffs by Farm Fresh's parent company, Supervalu Inc.

A 54-year-old city resident driving a minivan on U.S. 58 died after crossing into the opposite lane and hitting a school bus head-on during the morning commute Wednesday.

Jennifer Wester is a spokeswoman for the Virginia Department of Labor and Industry. Her name was incorrect in a story ("Golf course cited over worker's death") on Page 1 of Saturday's Hampton Roads section. Also, Cahoon Plantation golf course has 15 days to contest the citation from when the citation was received. The story misstated the deadline.
